The Diagnosis tab allows you to record all diagnoses that have been made for the patient as they relate to your practice. They might be diagnoses made by you or one of your therapists or by an individual outside your practice.

The Chart Notes tab allows you to record an unlimited number of notes for your patient. Chart Notes are assigned to one of three categories: Accounting Notes, Therapy Notes and Other Notes.
